Carriage Club Home Health provides a variety of skilled services to residents of Mecklenburg County. These services are provided in the comfort of your home, whether you live in a private home, or in an independent living or assisted living community. 

The home care services you receive depend on your individual needs and your doctor's orders. 

Some examples of services include:

Nursing: wound care after surgery or for ulcers that occur; teaching about new or changing medications; teaching how to give insulin injections; monthly catheter changes

Physical or occupational therapy: for strengthening and increasing range of motion after surgery; teaching transfer methods; teaching new or adaptive ways of doing activities of daily living; education on the use of new medical equipment. 

Speech therapy: working with swallowing disorders; relearning speech and other types of communication after stroke or other medical conditions.

Aides: assistance with bathing and personal care; assistance with transfers and/or ambulation.

Medical Social Workers: assessment and referral for long-range planning and decision making; education about or assistance with accessing community resources.

To qualify for Medicare or Medicaid covered Home Health services, the following criteria must be met :

You must be "homebound".    This may be a temporary situation, due to illness or surgery. (Homebound means the normal inability to leave home.  Leaving home requires a considerable and taxing effort, usually requiring assistance from others or devices.  Absences from home for non-medical reasons must be infrequent.)

A physician must order and manage your treatment plan.

You must have a need for Medicare or Medicaid covered skilled services.

Home Health services are intermittent care, not chronic or custodial care.

In addition to Medicare or Medicaid, Carriage Club Home Health also works with private insurance, workers' comp and private pay.

In the evenings, on weekends and holidays, our answering service will contact the nurse on call to assist you.

Companion Services

If you have a need for Certified Nursing Assistants or Companions, Carriage Club Companion Services may be able to help.  This private pay service is coordinated by a Registered Nurse, and is available to residents of Mecklenburg County.  Services may include light housekeeping, light meal preparation, grocery shopping, personal care, accompanying to medical appointments or other activities, companionship, etc.
